{"id":2446,"date":"2025-12-16T17:06:56","date_gmt":"2025-12-16T17:06:56","guid":{"rendered":"https:\/\/studenthub.elsevier.com\/en\/?p=2446"},"modified":"2026-02-06T19:11:13","modified_gmt":"2026-02-06T19:11:13","slug":"raising-concerns-about-patient-safety","status":"publish","type":"post","link":"https:\/\/studenthub.elsevier.com\/en\/blog\/raising-concerns-about-patient-safety\/","title":{"rendered":"Your Voice Matters: Raising Serious Concerns About Patient Safety"},"content":{"rendered":"\n<p><strong>By: Maria Ahmad<\/strong>, co-author, <em>Unlocking The Leader Within: A Medical Student&#8217;s Guide to Practical Leadership <\/em><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>A recent study estimated that over 237 million medication errors are made each year, just in England<sup>1<\/sup>. <em>237 million! <\/em>And that doesn\u2019t include other errors \u2013 mistakes made during operations, missed or incorrect diagnoses, unsafe discharges\u2026the list goes on.<\/p>\n\n\n\n<p>So, as a medical student or foundation doctor, chances are that you <em>will<\/em> witness someone somewhere about to make a mistake.<\/p>\n\n\n\n<p>How confident would you feel telling a scary consultant surgeon that they are about to operate on the wrong leg? Or telling the nurse-in-charge that they are about to give penicillin to a patient with an anaphylactic reaction to it?<\/p>\n\n\n\n<p>As daunting as it is, you <em>need<\/em> to use your leadership skills to raise these concerns because, for many patients, that could genuinely be the difference between life and death.<\/p>\n\n\n\n<p>But how? Worry not, we have a handy framework for you: <strong>PACE<\/strong>.<\/p>\n\n\n\n<p>PACE stands for <strong>Probe, Alert, Concern\/Challenge, Escalate\/Emergency<\/strong><sup>2<\/sup>. This is called a model of graded assertiveness which means that, as you go through the letters, you slowly give a more assertive response. PACE is a great model to raise concerns to those scary seniors to ensure patient safety while still sounding polite and respectful to your seniors.<\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><em><strong>Here&#8217;s an example for raising your concern to the nurse in charge who is about to give penicillin to a patient with anaphylaxis to it<\/strong><\/em>:<\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<figure class=\"wp-block-table is-style-stripes\"><table class=\"has-base-background-color has-background has-fixed-layout\"><tbody><tr><td><strong>Probe<\/strong> &#8211; &#8220;Have you checked the patient&#8217;s allergy status?&#8221;<\/td><\/tr><tr><td><strong>Alert<\/strong> &#8211; &#8220;I think the patient has had an anaphylactic reaction to penicillin before.&#8221;<\/td><\/tr><tr><td><strong>Challenge \/ Concern<\/strong> &#8211; &#8220;Tazocin contains penicillin, and this patient has had anaphylaxis to penicillin before. I am <strong>concerned<\/strong> it is unsafe to give the patient Tazocin.&#8221;<\/td><\/tr><tr><td><strong>Escalate \/ Emergency<\/strong> &#8211; &#8220;STOP! We cannot give this patient Tazocin. (Also highlight the issue to a senior).<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>By the time you\u2019ve reached the Challenge\/Concern stage, almost all seniors will be attentive to your concern, because you\u2019ve just highlighted the dangers to the patient three times in three different ways. If they are still proceeding with something dangerous, you need to stop them and get another senior involved.<\/p>\n\n\n\n<p>But what if you find out later that you were wrong? That\u2019s ok, use it as a learning opportunity and know that you were advocating for the best for your patient. It won\u2019t go unnoticed.<\/p>\n\n\n\n<p>So, next time you see someone about to make a mistake, use PACE to politely and assertively raise your concern.
