M\u00e1s informaci\u00f3n<strong><a href=\"https:\/\/studenthub.elsevier.com\/es\/transform-your-medical-education\/\"> aqu\u00ed<\/a><\/strong>.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Helicobacter Pylori, introducci\u00f3n esquem\u00e1tica<\/strong><\/h4>\n\n\n\n<h5 class=\"wp-block-heading\">Definici\u00f3n<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>Helicobacter pylori\u00a0<\/em>es una bacteria gramnegativa que coloniza de modo persistente el est\u00f3mago humano.<\/li>\n\n\n\n<li>La mayor\u00eda de las personas colonizadas por\u00a0<em>H. pylori\u00a0<\/em>se encuentran asintom\u00e1ticas, pero la presencia de\u00a0<em>H. pylori\u00a0<\/em>se asocia con un riesgo aumentado de ulceraci\u00f3n p\u00e9ptica y c\u00e1ncer g\u00e1strico.<\/li>\n\n\n\n<li>Cada vez hay m\u00e1s pruebas de los beneficios que se asocian a la colonizaci\u00f3n g\u00e1strica por\u00a0<em>H. pylori\u00a0<\/em>adem\u00e1s de riesgos de enfermedad conocidos.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Epidemiolog\u00eda<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>H. pylori\u00a0<\/em>se encuentra presente en individuos de todo el mundo, pero su prevalencia es variable y depende de la edad y la geograf\u00eda.<\/li>\n\n\n\n<li>La prevalencia de\u00a0<em>H. pylori\u00a0<\/em>en los pa\u00edses desarrollados ha disminuido a lo largo de las \u00faltimas d\u00e9cadas.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Microbiolog\u00eda<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li><em>H. pylori\u00a0<\/em>es una bacteria con forma de espiral, gramnegativa, ureasa-positiva.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Diagn\u00f3stico<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Los estudios diagn\u00f3sticos no invasivos son la serolog\u00eda, la prueba del aliento con urea y la prueba del ant\u00edgeno fecal.<\/li>\n\n\n\n<li>Las pruebas diagn\u00f3sticas invasivas son la endoscopia y el an\u00e1lisis de tejido g\u00e1strico.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Tratamiento<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>El tratamiento antimicrobiano solo est\u00e1 indicado en los pacientes con ulceraci\u00f3n p\u00e9ptica o en aquellos con un riesgo relativo elevado de sufrir un c\u00e1ncer g\u00e1strico.<\/li>\n\n\n\n<li>El tratamiento consiste en una combinaci\u00f3n de antimicrobianos y muchas veces incluye alg\u00fan f\u00e1rmaco capaz de reducir la acidez g\u00e1strica.<\/li>\n\n\n\n<li>\u2022Cada vez son m\u00e1s las cepas que presentan resistencia a los antibi\u00f3ticos. El conocimiento de la sensibilidad a los antimicrobianos permite optimizar las pautas de tratamiento.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Prevenci\u00f3n<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>La prevenci\u00f3n de la adquisici\u00f3n de\u00a0<em>H. pylori\u00a0<\/em>mediante inmunizaci\u00f3n no se practica de modo rutinario. Tras la adquisici\u00f3n de la bacteria, casi todos los individuos desarrollan colonizaci\u00f3n persistente de por vida. La colonizaci\u00f3n induce respuestas denominadas gastritis cr\u00f3nicas. Este proceso afecta a la fisiolog\u00eda g\u00e1strica, incluida la estructura glandular, la secreci\u00f3n \u00e1cida y el procesamiento de ant\u00edgenos que, a su vez, afecta al riesgo de enfermedad. La colonizaci\u00f3n por\u00a0H. pylori\u00a0aumenta el riesgo de ciertas enfermedades (\u00falcera duodenal, \u00falcera g\u00e1strica, adenocarcinoma g\u00e1strico no localizado en el cardias, linfomas de c\u00e9lulas B y, posiblemente, p\u00farpura trombocitop\u00e9nica idiop\u00e1tica), pero disminuye el riesgo de enfermedad por reflujo gastroesof\u00e1gico y sus complicaciones, incluido el es\u00f3fago de Barrett y el adenocarcinoma esof\u00e1gico o de cardias g\u00e1strico, as\u00ed como, posiblemente, el asma y enfermedades al\u00e9rgicas relacionadas de la infancia.<strong>Fuente:\u00a0<\/strong>Mandell, Douglas, Bennett. Cover, Timothy L.; Blaser, Martin J.\u00a9\u00a02021.<\/em><\/h6>\n\n\n\n<p><\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Enfermedad ulcerosa p\u00e9ptica<\/strong><\/h4>\n\n\n\n<p>La&nbsp;<strong>enfermedad ulcerosa p\u00e9ptica (EUP),<\/strong>&nbsp;definida como la presencia de ulceraciones en la mucosa g\u00e1strica y duodenal, est\u00e1 asociada a una importante morbilidad y mortalidad en todo el mundo. Casi el <strong>2% de la poblaci\u00f3n mundial <\/strong>padece \u00falceras p\u00e9pticas.<\/p>\n\n\n\n<p>La comprensi\u00f3n de la EUP cambi\u00f3 dr\u00e1sticamente a principios de los a\u00f1os 80 tras el descubrimiento de que&nbsp;<em><strong>Helicobacter pylori,&nbsp;<\/strong><\/em>una bacteria gramnegativa, era responsable de un gran n\u00famero de casos.&nbsp;<em>H. pylori<\/em>&nbsp;infecta el epitelio gastrointestinal y es uno de los pat\u00f3genos bacterianos m\u00e1s extendidos en todo el mundo, con mayor carga en los pa\u00edses&nbsp;en desarrollo. La infecci\u00f3n suele producirse en las primeras etapas de la vida. Dado que&nbsp;<em>H. pylori<\/em>&nbsp;puede sobrevivir al entorno \u00e1cido del est\u00f3mago, se calcula que m\u00e1s de la mitad de la poblaci\u00f3n mundial est\u00e1 infectada, con una prevalencia del 30 al 40% en EE. UU. Adem\u00e1s, la actividad ure\u00e1sica de&nbsp;<em>H. pylori<\/em>&nbsp;puede producir amoniaco, lo que impide que las gl\u00e1ndulas del est\u00f3mago perciban el verdadero pH g\u00e1strico, lo que conduce a una liberaci\u00f3n inadecuada de gastrina. La hipergastrinemia resultante promueve la hiperplasia de las c\u00e9lulas parietales secretoras de \u00e1cido y el aumento de la secreci\u00f3n de \u00e1cido.
